Teen Survey/Entry Form

Rules

 

To enter:

1.  Print out the Teen Survey & fill it in.

2.  Turn it in at your Harris County Branch Library.

 

Reliant Energy and the Houston Texans Foundation are providing free tickets to Harris County Public Library for the Power Readers Program.  The tickets are for two home games:  Houston Texans vs Indianapolis Colts on December 12 at noon and Houston Texans vs Cleveland Browns on January 2 at noon.

These tickets will be given away in pairs at each of the Harris County Public Libraries.  To enter, fill out the Teen Survey and turn it in to the staff at the Circulation Desk of your library.

Reliant Energy is donating the tickets and the Power Reader t-shirts.

Aramark, the official food vendor of Reliant Stadium is donating hot dogs and drinks.

 

Rules:

1. Only one entry per person.   Duplicate entries will be thrown out.    You can enter at only one Harris County Branch Library.
2. You must be 12-17 years old to enter.
3. Tickets may not be sold or exchanged.
4. Winners will receive two tickets—one for the teen winner and one for an adult to accompany winner to the game.   An adult must accompany each teen winner to the game.
5. Winners must provide their own transportation to the game.
6. Parking is not included.
7. No additional tickets will be available from the library.
8. Tickets are for the Power Readers section at the game and alcohol will NOT be allowed in this section.
9. Information on the surveys may be used for future teen programming at the library.
10. Winners will be contacted by phone.
